Assessments
Services begin when you are assigned to a care coordinator who is trained to organize and manage your specific needs. They will personally follow your case throughout the entire time of service.
Our registered nurse will do a complete diagnostic assessment based on the physician's Plan of Treatment, as well as client and family needs in order to create an individual Plan of Care. The nurse will then instruct and supervise the
assigned home health aide on the necessary care.
Our nurses will provide re-assessments on a regular basis and maintain constant communication with the physician, family and agency.
Care Coordination
Our geriatric care managers and care coordinators are responsible for managing and coordinating various services that may be involved in the client's home care.
- Constant communication to keep the client and their family informed
- Scheduling home visits with therapists and nutritionists
- Arrangement for the delivery of medical equipment and supplies (i.e. wheelchairs, commodes, walkers, hospital beds and diapers)
- Continuity of care throughout transitions (between home, hospital and assisted living facilities).
Disease Management & Monitoring
Disease management and monitoring is incorporated into our care, reducing healthcare costs and improving quality of life for individuals with chronic diseases

In Home Care
Home health aides are available to provide care on an hourly or live-in schedule.
Our staff is available 24 hours a day, 7 days a week.
- Assistance with daily activities such as cooking, eating, dressing, bathing, grooming and toileting
- Assisted movement
- Help with activities such as shopping, driving, housework, paying bills and laundry
- Accompaniment to doctor appointments Assistance with the administration of medications
- Socialization and companionship assistance while traveling
Customized Food Preparation
Meals are prepared according to dietary and religious needs. Al of our aides can provide home cooking and are
also trained in keeping a Kosher home
Housekeeping & Safety
- Housekeeping and household organization

- Home safety assessments and recommendations to improve home environment
- Training in emergency preparedness
- Agency will help arrange home maintenance services such as painting and plumbing
Wellness and Social Support
- Enhanced socialization

- Facilitating special interests such as the languages, arts, music and politics
- Exercise activities
- Improving diet and nutrition

- Help with problem solving around options for care
- Educating client and family on home care needs related to specific healthcare issues
- Advocacy for the treatment of mental health problems including depression, substance abuse and abusive behaviors
- Reduction of social isolation through the introduction of specialized activities, companions or the use of community services
- Working closely with clients and family to overcome resistance to care
