Home Care for Individuals with Chronic and Complex Illness
- Such as Alzheimer’s disease, dementia, constructive heart failure, diabetes, physical and mental disabilities.
Independent Living Program
- Our team is focused on maintaining the client’s independence as much as possible within a safe environment
- Aides will go shopping with or for the client, take them to doctor appointments, cook desired meals and engage them in indoor and outdoor activities to promote physical, mental, emotional and social wellness.
Superior Supervision
- Our trained, interviewed and screened home health aides are continuously oriented and supervised by registered nurses.
Integrated Care
- Our nurses and aides all work toward the same goal in providing superior medical management
- Care services are based on assessments, evaluations, monitoring and care coordination
